Use this service to register to use GP online services.
All patients wishing to use this service need to apply individually. Filling this form out does not automatically give you access, it will be at the discretion of the surgery.
Upon receipt of the form it usually takes up to 7 working days for us to action your request. You will receive your username and password either via SMS or email.
You can use this service if you:
- are registered at the surgery
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
- photographic identification and proof of address
A parent or guardian will need to be registered for the online services already in order for us to register a child as this will be linked to their account. When the child reaches 13 years of age, this access will be automatically removed. Those children aged 13 to 16 years may need a Gillick competency assessment before being given their own access or allowing a parent or guardian to continue having access. When a child reaches 16 years of age, they will be able to register for their own online services account.
